Getting to Know Your Health
After a quick blood draw, biometric screening (don’t worry, we’ll fill you in on that) and questionnaire, we’ll have all the information we need to help you maximize your health.
Your Health Questionnaire
Also called a health risk assessment (HRA), it asks questions about your personal and family health history, the types of foods and physical activities you enjoy, and other questions about sleep and stress.
It shows us how wellness presently fits into your life and what family predispositions you may have for certain conditions. It also gives us direction for food and exercise recommendations that you’d enjoy.
Your Biometric Screening
This sounds more complicated than it really is—but it’s still really important.
We take measurements of your height, weight, blood pressure and other physical measurements.
It allows us to calculate your body mass index. We use this information to set a baseline for your physical condition and track your fitness success.
Your Blood Draw
With this blood sample, we’ll be able to run a host of sophisticated lab tests that can find your risk for developing serious conditions like heart disease and diabetes.
It also provides a scientific basis for health improvements.
The tests indicate simple but beneficial changes you may need to make in order to maintain a healthy lifestyle.
Knowing yourself is a great starting point.
So what happens with the information that we get from all of your tests?
It’s used to build your one-of-a-kind personal health profile. It shows your health risks and contains links to helpful health-related content based on your results. Your profile also offers goal tracking, game features, chat and a whole lot more.
What are the most important parts of my personal health profile?
Your lab report is the key to your profile. It’s a breakdown of your health risks with a handy summarized chart, organized according to this easy system—green is “good,” and red is “risky.”

No. Health information is extremely personal and yours will be kept 100 percent private as per state and federal HIPAA laws to protect your health information. Your dashboard employs data encryption and is continuously monitored to detect threats to your privacy.

This is it. Please fill out this form on your dashboard as completely and honestly as you can.

Step III—Schedule Your Blood Draw and Biometric Screening
We do these all at once. So try to choose a time when your workload is predictable so you won’t have to drop everything to put out fires and herd cats.
Unless of course you are a firefighter or professional cat herder, in which case, carry on.
Before Your Appointment
Be sure to fast before your blood draw—it makes your results far more accurate. If you take daily medications, please take them as directed with plenty of water!
What to Bring to Your Appointment
When you go to your blood draw and biometric screening, please bring a printed copy of the form that you filled out when you signed up online. Wear comfortable clothing with socks and shoes that are easy to take off.
